Our Family Medicine physicians and nurse practitioners recommend that you have a well child checkup for your child (ages 2-17) if it has been more than one year since the child's last well child exam. Please verify your health insurance plan benefits prior to making an appointment as some insurances only pay for it once every two years or don't have this as a covered benefit at all.

Just a reminder to schedule your child's sports physical / back to school physical as soon as possible during the summer break. Most of the fall sports start in mid-August so it is a good idea to schedule these appointments early in the summer. Family Wellness Center has a supply of sports physical forms or you can bring your own specific school district sports physical form. Please complete the patient sections on the form before coming to the office. PLEASE NOTE: in some instances Family Wellness Center may not be able to do just a sports physical for your child (ie. if your child is not a current patient at FWC, or hasn't had a full well child check up within the last 1-2 years).

We have a cash discounted sports physical fee offered to established FWC patients only. The child's sports physical only is $25.00 cash price, or we can complete a sports physical form at the time of a full Well Child check up and will bill your insurance for a full well child checkup if you have this routine child preventive care benefit on your insurance plan.

Please bring the child's immunization record with you to the appointment so we may verify which shots your child may need and update the record as needed. Having a current list of your child's immunizations will prepare you in the event that your child's school requests a copy.
